TATTOO CONSENT FORM

Please read and sign below to indicate that you understand the implications of getting a tattoo.
In consideration of receiving a tattoo from the following: (Artist), I agree to the following.
- That I have been fully informed of the risks associated with getting a tattoo. I understand these risks can lead to injury, including but not limited to, infection, scarring, difficulties in detecting melanoma and allergic reactions to tattoo pigment, latex gloves, and soap. Having been informed of the potential risks associated I wish to proceed with the tattoo application and I freely accept any and all risks that may arise.
- TO WAIVE AND RELEASE, to the fullest extent permitted by law, of the artist from all liability whatsoever, for any and all claims or causes of action that I, my estate, heirs, executors or assigns my have for personal injury or otherwise. Including any direct and/or consequential damages which result from the application of my tattoo whether caused by the negligence or fault of the artist.
- That the artist has given me the full opportunity to ask any and all questions about the application of my tattoo, and all of my questions have been answered to my total satisfaction.
- The artist has given me instructions on the care of my tattoo while it is healing and I understand them and will follow them. I acknowledge that my tattoo is prone to infection if not properly cared for. If touch up work is needed due to my own negligence, I agree that the work will be done at the discrepancy of the artist and may incur a fee. Touch ups are free of charge for certain areas up to 6 months from the time of application. (Excludes hands/feet)
- A tattoo is a permanent change to my appearance and can only be removed by laser or surgical means. Which possibly can be disfiguring and/or costly, and which in all likelihood will not result in the restoration of my skin to its previous appearance.
- The artist is not responsible for the meaning or spelling of the symbol, text or design that I have provided to them.
- Variations in the colour and design may exist between the art I have selected and the actual tattoo when it is applied to my body.
- I release all rights to any photographs taken of me or the tattoo, and give consent in advance to their reproduction in print or electronic form for use by the artist or studio.
- I agree to reimburse the artist for any legal fees and costs incurred in any legal action I bring against either the artist or company, and in which either the artist or company is the prevailing party. I agree that the Courts of WA, Australia shall have personal jurisdiction and the venue over me, and shall have exclusive jurisdiction for the purpose of litigating any dispute arising out of, or relating to this agreement.
I acknowledge that I have been given the adequate opportunity to read and understand that I am signing a legal contract waiving certain rights to recover against the artist and company.
- I have eaten in the last 2 hours.
- I have not consumed drugs or alcohol in the last 24 hours.
- I have no heart conditions.
- My blood pressure is regular.
- I do not have any conditions that compromise my immune system.
- I have never suffered from hepatitis A, B or C.
- I am not diabetic.
- I do not have any blood clotting disorders.
- I am not pregnant or breastfeeding
- I do not suffer from any form of seizure causing condition.
- I understand that reaction to the pigment is still possible, even after the tattoo is healed.
- I understand that every care has been taken to ensure that this procedure will be carried out in a hygienic manner, and the aftercare of the tattoo is my sole responsibility.
- I consent to any actions or conduct of the artist, representatives or employees of the tattoo shop necessary to perform the procedure.
I Hereby declare that I am of legal age (18 years or older) for a tattoo, and have provided valid proof of age and am competent to sign this agreement.
